I, the undersigned, being the applicant coach / manager or parent/guardian of the team, hereby confirm that permission for the team players to play in the Pittwater Classic has been granted by the parent/guardian. The applicant agrees to be bound by the rules of the competition and indemnifies the club, its officers, members and servants against any claim and compensation for any loss or injury substained during a Pittwater Classic Competition. I agree to pay the team registration fee of $550 + GST Payment details due within 30 days prior to the event.
